Miadowicz, PT was removed from the authorized independent medical exam list effective March 31, 2008, because her license to practice physical therapy in Oregon "lapsed" on that date. No complaints or discipline from the Workers' Compensation Division were involved in this action.     On June 30, 2008, Norman D. Rabin, DC was removed from the authorized independent medical exam list effective April 30, 2008, because his license to practice chiropractic medicine in Oregon was not renewed. No complaints or discipline from the Workers' Compensation Division were involved in this action.     John Carter, MD was removed from the authorized independent medical exam list effective March 7, 2008, at the doctor's request. No complaints or discipline from the Workers' Compensation Division were involved in this action.   Thomas D. Caruso, DO was removed from the authorized independent medical exam list effective December 31, 2007, because his license to practice medicine in Oregon "lapsed." No complaints or discipline from the Workers' Compensation Division were involved in this action. John S. Corson, MD was removed from the authorized independent medical exam list effective January 10, 2008, because he retired his license to practice medicine in Oregon effective that date.
