All employers in Oregon who are required to provide workers' compensation
coverage must display a Notice of Compliance poster. Use this form
to order additional copies of the Notice of Compliance poster.

When you need notarized information on workers’ compensation
coverage dates for an employer, use this form.
Request
for hearing form
OAR 436-001-0019 prescribes the requirements for requesting a hearing.
Use of Form 2839 is optional. However, completion of this form ensures
your request is complete.
