Effective
Jan. 2, 2008, naturopathic physicians must certify to the director
of the Department of Consumer & Business Services that they
have reviewed and read certain informational material provided by
the Workers' Compensation Division, before they treat any patients
with Oregon workers' compensation claims.
| | | | | If you have not certified to the director and treat a patient with an Oregon workers' compensation claim on or after Jan. 2, 2008, the insurer will not have to pay for the services you provided. | | |

physician guide to Oregon on-the-job injuries
An introduction to the American College of Occupational and Environmental Medicine's disability prevention guidelines titled, "Preventing Needless Work Disability by Helping People Stay Employed"
The Employer-at-Injury Program (return to work) information
After you have read the above materials, please complete the "Statement of Certification" online form. |
