Nurse practitioners authorization

Effective January 1, 2004, the authority of licensed nurse practitioners to treat Oregon injured workers was expanded, as set forth in ORS 656.245 and Oregon Administrative Rules Chapter 436, Divisions 010, 015, and 060.

Beginning on October 1, 2004, this authorization applies to Oregon licensed nurse practitioners only if they certify they have reviewed and understand informational materials provided by the Workers' Compensation Division (WCD) about the workers' compensation system.

If the director has not authorized you and you treat a patient with an Oregon workers' compensation claim, the insurer will not have to pay for the services you provided.

If you have questions, please contact Rachel Myllenbeck,
503-947-7747.

The required materials include:
Nurse Practitioners guide to Oregon on-the-job injuries

An introduction to the American College of Occupational and Environmental Medicine's disability prevention guidelines titled, "Preventing Needless Work Disability by Helping People Stay Employed"

The Employer-at-Injury Program (return to work) information

After you have read the above materials, please complete the "Statement of Authorization" online form.
